2.  Make check out to "Eagle Rock Chamber of Commerce" (refer to fee schedule, below).
3. Mail application and check to the chamber. Mailing address is on the application.

Annual Fee Schedule:
Business Employing more than 10:  $262.50
Business Employing 4-9:                    $187.50
Business Employing Up to 3:             $150.00
Non-Profit:                                            $112.50
Residential or Family:                          $97.50
